Audit

This week has been doing audit. Today was the worst result of all because I guess everyone do not expect auditor to go to their ward on Saturday.
Some staff were rude, some were defensive and some dont even bother to take a second look at me.
Well, fine, I do my audit still but more strict on these people.

Scene 1:
Me: Where is your goggle?
Senior staff nurse: Wait, I want to go toilet.
After few minutes I saw the same staff again.
Me: I was asking you where is your goggle?
SSN: Here lor (point to the drawer). Any other question?
Me: All right, since you ask tell me the 5 moments of hand hygiene.
(It's 1) before patient contact 2) after patient contact 3) before aseptic task 4) after contact patient's surrounding 5) after body fluid exposure risk)
SSN: Errr before procedure, after procedure, before and after touching body fluid, after touching...
Me: Wait... before what procedure? and before or after touching body fluid?
SSN: Before and after procedure lah.
Me: No, what procedure? Before a what procedure? Is after a procedure part of the 5 moments? then contact with body fluid do you wash your hands before or after?
SSN: Before and after lah, correct mah! Before and after procedure, before and after body fluid...before and after you touch the patient lah.
Me: Like that is how many moments????? Do you have the chart in the ward? Can you point to me and read all out.
She brought me to the chart and said
SSN: Correct lah before and after procedure...
Me: Can you see properly? It's before and after patient contact, before aseptic procedure and after touching body fluid and surrounding.
SSN: Correct lah I said that mah!
Me: NO, you said before and after body fluid, before and after procedure that is not right.
SSN: Ai ya whatever lah I dont want to argue with you.
Me: I am not trying to argue, you did not get it right at all. You should know everything by now because audit is everyday.
SSN: Never mind lah ok ok I dont want to argue. Any other question.
Me: Ok since you ask, show me how u enter patient temperature.
SSN: (click and go in)...
Me: Ok, at what time do you enter into the system if a patient having fever?
SSN: Uh... what what time? Two times a day lor.
Me: Yes, then at what time?
SSN: About 8am and 2pm lah
Me: Wrong.
SSN: Correct what, we always enter about 8plus or 2plus.
Me: Anyone know the answer?
EN: It's at 4am and 4pm.
Me: Correct. Then which temperature do you enter?
EN: The highest temperature.
Me: Correct. Then when patient no longer having fever then do you still need to enter.
EN: Yes, for another 48hours.
Me: Yes. SSN, this is the answers that I want. I only ask things that YOU SHOULD KNOW and not question that I invented. You have to know what you suppose to know.
She called herself a senior staff nurse!??!?! EN can do better than her!

Scene two:
Me: Can name me the 5 moments of hand hygiene?
SN: (Do not know then turn and look at the chart)...
Me: By now you should know already, shouldnt be looking at the chart.
SN: You didnt say cannot look what. Before and after patient contact.....blah blah....
Me: Ok now that you have read and saw, I want you to look away and repeat.
SN: I still dont know. (Look at the chart to repeat)....
What is this????!

Scene three:
Everyone was very caution when they see auditor but this particular SN dont bother. She touched the patient's hand, lifted the patient up, then served the patient's diet followed by touching the curtain. She then walked out the patient's room. I approached her and asked her whether she saw me observing her she said yes. I asked her then how come she didnt use handrub after contacting patient's surrounding. She replied "Oh I forgot"... then continued to serve other patient's diet.
A just dont bother attitute lor....

Very bad experience today!
