"Dyspnea" night
My first night not exactly crazy but patients were all breathless! Took over one NSTEMI with very bad pneumonia co-manage patient at 12MN. Came to us just for monitoring (as usual) however he has more of the breathing issue than cardiac problem. With 100% O2, his saturation was only 82%, he looked all right in the beginning but tired out at the end of our shift.
One of the old lady complained of breathlessness since we started our shift but her saturation was 97%. The whole night she was calling out to us "MISSY AH..... MISSY.... MMMMmmmIIISSSSYYYYYY "chuan" ah....." We dont know how to treat her because her parameters seems normal. Towards the middle of the night, her saturation dropped so we did an ABG, it indeed got worst but not to the extend for intubation.
Another male patient was on BIPAP for a couple of days. I can see that he is deterioriating last night and he himself was complaining of difficulty in breathing. He was very breathless and sweaty towards early morning. We kept saying that he will be the next one to be intubated and true enough the team doctor ordered intubation in the morning round.
There were two intubated patients last night and both are quite sick, I wonder how many do I get tonight??? Tonight is the "jinx" registrar on......... SIGH.................
